However it really depends upon the state. You may need anything from a test to a coupple of extra classes.

depends on the district you end up working for, and what school you're placed in. I use to teach math at a middle school in Palmdale, CA, and I can tell you I had to teach 2nd grade math to those neanderthal kids! The fact that you may want to teach in a middle school setting doesn't mean you'll actually teach grade level curriculum. I had students who could barely write their names, much less multiply simple numbers! And no, they weren't the special ed students, they were the general population students. Be prepared for anything. You may want to teach math, and they may place you in an English classroom! That's the truth about teaching nowadays.

Why would you teach at school for a ridiculous salary if you have masters in applied math? As for the shortage of math teachers in US - these days when the economy is crashing and school districts laying off hundreds of teachers - it is questionable. I am a high school teacher in Miami. The requirements here are different, but it would be possible for you to start teaching with a Temporary Certificate, and then complete all the requirements for Professional certificate - maybe, through the alternative certification process, while working at school: taking Professional Development courses, creating a portfolio, etc.
