I would contact your local Health Department. If they cannot help you, they should be able to direct someone who may be able too. Also check to see if the hospitals in your area offer these services. I know one of our local hospital does this sort of thing all the time.

What age is the child?
For my son with DS who is 3 I focus mostly on speech and fine motor.
For speech I do a lot of matching pictures and objects, colours, etc. Also use sign language and mirrors with dress ups and hats and things like that so he touches and feels the things I am teaching him about.
Fine motor is everything to help strengthen his hands to help with scissor cutting and writing, pegs, jars, toys with buttons, hair clips, play doh, I could go on.
Hope this is a little helpful, but I have a feeling this child is school age and maybe a bit beyond these suggestions. But I do know that with reading, apparently generally speaking, these kids learn to read best by learning whole words rather than letter by letter. That is just something we have been told by my Down Syndrome Society.
